The Civil Service Commission (CSC) has officially adopted CS Form No. 212, Revised 2026 (Personal Data Sheet) through CSC Memorandum Circular No. 05, s. 2026. This revised version supersedes the 2025 edition and restores the Monthly Salary and Salary/Job/Pay Grade and Step Increment columns in the Work Experience section to ensure the completeness of personnel records.
